2024-2025 Bubblers Athletics Preparticipation Forms
2024-25 SMSD Athletic Preparticipation Forms (1).pdfStudents interested in competing on an interscholastic junior high level and/ or high school level athletic team are required to complete and submit the Bubblers Athletics Preparticipation Forms. Eligibility is reviewed and pending receipt of all required PIAA and South Middleton School District documents, students will be provided clearance to begin participation and roster consideration.
