2023-24 SMSD Athletic Pre-Participation Forms.pdf

Students interested in competing on an interscholastic junior high level and/ or high school level athletic team are required to complete and submit the Bubblers Athletics Preparticipation Forms. Eligibility is reviewed and pending receipt of all required PIAA and South Middleton School District documents, students will be provided clearance to begin participation and roster consideration.

The Bubblers Athletic Recertification are a PIAA and SMSD requirement for consideration and eligibility for any interested student wishing to pursue an athletic program. The recertification form is utilized for any student-athlete who submitted the Preparticipation form at the start of the school year and seeking participation on additional athletic teams.

Board Approved Athletic Staff are required to complete all of the necessary certifications and clearances. This list will be presented to athletic staff, volunteer and paid, as part of our onboarding procedures.

The Athletics Emergency Standards is provided to the athletic coaching staff and outlines a process when addressing an emergency. The standards are reviewed prior to the start of each athletic season to support effective response to situations that may arise during an athletic event.

Athletics Transportation SOP.pdf

The Athletic Department supports transportation to and from athletic contests. Please review the athletic transportation standard operational procedures regarding necessary form submission in the event that your student-athlete needs to obtain alternative transportation to or from an away contest.

Transportation Waiver Form.pdf

The Athletics Transportation Waiver Form is required to be submitted 24 hours (minimum) prior to a competition date that a student-athlete is seeking non-district issued transportation. Please review the Athletics Transportation SOP for additional information.

FBI Clearances Directions.pdf

Board approved PAID athletic staff are required to complete and submit their FBI clearances every 5 years. Board approved VOLUNTEER athletic staff are required to complete and submit their FBI clearances IF they have not been a resident of Pennsylvania for the past 10 years.

Pennsylvania State Background Clearances Directions.pdf

Board approved athletic staff (Paid and Volunteer) are required to complete and submit the PA State Clearances every 5 years from the date of their appointment.
